Leadership Team

Administrator: Jeremy Frehner
Picture
Jeremy is an Alumni of Dixie College and Southern Utah University and has a bachelor's of Health Care Administration. He was a Physical Therapist for 20 years before he became the Administrator of Seasons Health and Rehabilitation.

He has traveled to all 50 states, has run 75 marathons throughout the United States and has even participated in an Iron-man event. He loves gardening, swimming, hiking, traveling, camping, and backpacking. 

He is a happy husband, father and grandfather. He considers this his greatest achievement. "I love everyday life and try to make it my best day ever." He loves serving others, helping those in need and making people better in same way.  
Director of Nursing: Terral Bassett

Terrel graduated from Dixie College in Nursing in 2000 and is an RN and WCC, Would Care Certified. She was chosen as the top registered nurse in St. George, Utah by the International Nursing Association. She also owns and manages Ridgeview CNA School in St. George.

She loves camping, fishing, 4-wheeling, and teaching. She has been married for 20 years, has 4 children and 2 grandsons, who are the love of her life. 

"I love being a nurse and caregiver. I love to help others. I love being the Director of Nursing and working at Seasons. This is an amazing place. I feel blessed daily, working side-by-side with all of my co-workers. My favorite word  is empathy. I love teaching it to others. I love being a mentor, teacher, advocate, boss, learner, healer, friend, listener and empathetic."
Resident Advocate: Sara Kelley

Sara has 4 children, one of which is a CNA at Seasons. She collects funny hats that keep the residents entertained. She likes photography, scenic drives, her ferret Jerry, and 2 dogs Marley and Roxie. 

"I love knowing that I'm helping in our facility and in the community."

Her many responsibilities include:
  • Addressing all admission questions
  • Working with Social Workers, doctors and hospitals
  • She specializes in helping with insurance questions and needs including Medicare and Medicaid
  • She aligns discharges and makes sure that Home Healthcare is received if a resident returns to their home
  • She works within the facility to assist in solving any problems and concerns for residences and their families, whether that is resolving financial issues, grievances or other issues. 
Office Manager: Jane Lasko

Jane has worked as the Office Manager at Seasons for 6 1/2 years. She keeps our facility running smoothly and is the best of the best at her job. 

"I love working at Seasons. I consider is a gift from Heaven to be able to associate with the Seasons Family."

She's been married for 56 years and has 5 children and 16 grand-children. ("Some of my kids are slackers in that department.")

Her hobbies are counted cross stitching, gum paste, flower making, and quilling. She made the beautiful picture above our mantel in the entryway.
Physical Therapist: Russell Tolbert
Picture
Russell graduated from the University of Central Arkansas with a degree of Physical Therapy. Over the 28 years as a Physical Therapist he has worked in a variety of settings including impatient acute hospital care, acute rehab, transitional care unit, home health and skilled nursing rehab. The last 14 years had been spent working in skilled nursing facilities which is the setting he has found to be the most rewarding. 

Physical Therapy focuses on the treatment of disease, injury or the prevention of loss of mobility through wellness programs using interventions to improve range of motion, strength, posture, endurance, balance, coordination, joint mobility and alleviating pain to help an individual regain their independence with functional mobility skills. 

Physical Therapists are trained to assist in the following areas: ​
  • Gait and stair assessment and training
  • Balance assessment and training with fall risk management
  • Therapeutic exercise programs to improve range of motion, strength and endurance
  • Pain management with the use of modalities including massage, heat/cold therapy, ultrasound, electrical stimulation and manual techniques
  • Home assessments to ensure a safe return home, alone or with family, along with recommendations for home modifications or equipment needs
  • Patient and family education to ensure safety with post treatment programs when discharged home
Occupational Therapist: David Gray

David is a graduate of Utah State University with a Bachelor of Science and the University of Utah with a Master's Degree in Occupational Therapy.

He has been married for 14 years and has 2 children. 6 and 12. He loves football, baseball, golf, pickle-ball, hiking, and camping with family and friends. His new favorite hobby is ice fishing.

"I love to help people feel better and live a more full, healthy and safe life."

Occupational Therapists help people develop skills for daily living such as:
  • Bathing, dressing, grooming, toileting, eating, etc
  • They also help with instrumental activities such as managing shopping, cooking, medications, housework, laundry, driving, using public transportation, using the phone, and finances so their clients can participate more fully in the life they choose or prevent a disruption in their day to day lives.

Speech Pathologist: Matthew Hess
Picture
Matt graduated with honors from the University of Utah with a Bachelor degree in Speech and Hearing Science and from Idaho State University with a Master's degree in Speech Language Pathology. He has been in this field for 18 years. 

Matt loves chocolate, exercising, hiking and playing with his dog. He loves seeing his patient's personalities and seeing them improve.

He helps patient's with:
  • Swallowing problems
  • Communications problems 
  • Cognitive problems
CNA Supervisor: Jessica Mock

Jessica received her CNA license in 2007 and has worked at Seasons for 13 years. She has been CNA Coordinator for over 7 years. She enjoys camping, fishing, spending time with family and friends. She has a 12 year-old, charming daughter.

"There is nothing more fulfilling than helping others, and knowing at the end of the day you made a difference for someone. Our residents are all a part of my family and I love them all dearly."
Restorative Assistant: Corbin Garrett

Corbin has been a CNA for 4 years. "After having tried many different types of jobs I realized how much I love working in nursing. I love being a CNA, the people I work with and for, and the privilege of caring for our residents."

His interests are typical guy stuff: gaming, motorcycling, off-roading and also cooking. The following is a thought that represents his philosophy:

"Why nursing? They're the ones who open the eyes of a newborn and gently close the eyes of a dying man. It is indeed a privilege to be the first and last to witness the beginning and the end of life."

We offer a range of restorative programs including:
  • Passive Range of Motion (PROM)
  • Active Range of Motion (AROM)
  • Splint or Brace Assistance
  • Transfers
  • Ambulation
  • Dressing and Grooming
  • Eating and Swallowing
Transportation Manager: Lisa Panici
Picture
Lisa is a CNA and wears many hats at Seasons, assisting with anything that needs to be done. Lisa Loves to smile and laugh. Her phone ringer is a mooing cow. Coffee makes her so happy. She loves camping, knitting, sewing, coloring and watching TV. She was born and raised in Illinois. She has been married for 26 years. She says that "I love my family with all my heart~ Smiling and hugging people every day fills my heart and soul."

The Transportation Department is responsible for the following:
  • Scheduling doctor and dentist appointments as well as any other health care appointments
  • Transport residents to and from appointments and remain with them
  • Pick up new residents

Assistant Transportation: Jennifer Filice

Jennifer has been a CNA for 8 years, has worked at Seasons for 4 years, and is currently working towards her LPN. Her mother was also a CNA and encouraged her in that direction. She likes helping people. She stated that it always comes back to her in return. She feels that Seasons' staff and residents are her family. 
She loves the outdoors, camping, fishing and hunting. She loves being with her parents and family. They mean the world to her. She is the mother of 3 and grandmother of 1. 
Recreational Therapy Director: Michelle Anderson
Michelle has been a licensed Therapeutic Recreational Technician (TRT) for 27 years, 11 of which have been at Seasons Health and Rehab. She loves spending time with family, hiking, playing games, traveling, and listening to uplifting music. She is an artist and enjoys drawing, painting in watercolors and acrylic mediums. She also enjoys reading the scriptures, being with her dogs, gardening, antique shopping, thrifting and re-selling her finds. 

"My job is my life calling. I have found joy from being able to serve others and our residents at Seasons. I have been humbled over the years to have this opportunity. I love my job. The residents and staff have all become my family and I love each and every one of them. I am so blessed an grateful to have them in my life." 
Dietary Coordinator: Yanely Santos

Yanely has worked in a dietary department for 21 years and at Seasons for 14 years. She is currently attending school to become a Certified Dietary Manager. Yanely loves to camp, hike, and dance. She enjoys seeing the residents happy with their meals.
  • She assesses residents so that her staff can help with dietary needs
  • She teaches different menus to her staff
  • Looks for ways to prepare better quality meals
Health Unit Coordinator: Ashley Davis

Ashley began working at Seasons in 2008. She has worked as a CNA, restorative aide, CNA supervisor, social work, medical records, and central supply. Her current position entails focusing on:
  • Doctor's orders
  • Lab coordination
  • X-rays
  • Admission orders, discharges, transfers, financials
  • Assisting any department heads
  • Ordering supplies
"Healthcare is my passion! Everyday I get to be someone's friend, hold someone's hand or even to be someone's shoulder to cry on. I make lasting relationships with the residents and their families that last after their stay with us. I'm so blessed to be here: the residents don't realize how much they bless my life! I love them!" 
MDS Coordinator: Carl Green
Picture
Carl is the Nurse Assessment Coordinator. He is responsible for submitting resident health information to the government. 

"I enjoy being able to bring support and happiness to others. I enjoy hearing about the lives others have led."

Carl has a BS in Nursing, RN from Grand Canyon University in Arizona. He has been married to Diana for 17 years, has 7 children and 4 grandchildren. 

He served a mission for the Church of Jesus Christ of Latter-day Saints in St. Louis, MO. 

Through birth and marriage, he has 20 siblings.

He enjoys videos, reading, raising chickens, baking, camping and cleaning. His dream job is to become an author of fantasy and scifi books. 
Billing Manager: Tawndi Barrett

Tawndi is a CNA and began her career at the old Porter's (now Seasons) in 2002. She became transport manager for 6 years until 2019 when she became the Billing manager. 

Her Role at Seasons has many layers;
  • Computing balances
  • Recording billing payments
  • Compiling account statements
  • Billing Medicare and Medicaid
  • Check Insurances for admissions and therapies
  • Collecting card payments, maintaining up-to-date records of patient's demographics
  • Process bills for third party insurances
Tawndi has been married for 13 years. Together they have 6 children, one of whom is a CNA at Seasons. She was born and raised in Oregon and lived 2.5 years in the Cayman Islands where her husband was raised. She loves to sew, craft, go on road trips, camp, but her favorite is fishing. 
Infection Preventionist Nurse: Jeri Orr

  • Jeri has been in healthcare for 34 years. She got her LPN in 2000 when she graduated from Dixie College. Jeri keeps our building safe. Her foremost responsibilities are: 
  • Monitoring resident's infection rate
  • Keeps staff trained in regards to infection prevention and transmission
  • Responsible for residents and staff to stay current on their immunizations
  • Keeps up to date with current Covied-19 status and ensures that residents and staff are well informed

Jeri is happily married to a wonderful man. Together they have 7 children and 14 grandchildren. She enjoys scrap-booking and riding on her husband's Harley with him. She enjoyed participating in rodeos when she was in high school. Jeri says that "This is the best job ever! I love to do things for people that they can't do for themselves. My favorite clientele is working with Alzheimer's and dementia patients."
House Keeping: Maria Martinez

Maria has been married 39 years. She has 3 children, 12 grandchildren and 1 great-grandchild. She likes to cook for family, enjoys celebrating all occasions, and loves to dance.

"I love to work at Seasons because it is the best place to work, with the best staff, and has a family environment."
Maintenance Coordinator: Marty Maggard

Marty has worked in Mechanical Maintenance all of his adult life. He has worked at Seasons for 9 1/2 years. He keeps our building in excellent working condition to meet the Federal codes and guidelines. His main focus at our facility is life safety. He fixes everything and anything. We couldn't do without him. 

Marty has been married to Debbie, (who works in Recreational Therapy) for 38 years, he has 5 children and 6 grand-children.

His hobbies include fishing, hunting and metal detecting.
