Jeremy Frehner, Administrator 
Picture
Jeremy is an Alumni of Dixie College and Southern Utah University and has a bachelor's of Health Care Administration. He was a Physical Therapist for 20 years before he became the Administrator of Seasons Health and Rehabilitation.

He has travelled to all 50 states, has run 75 marathons throughout the United States and has even participated in an Ironman event. He loves gardening, swimming, hiking, traveling, camping, and backpacking. 

He is a happy husband, father and grandfather. He considers this his greatest achievement. 

"I love everyday life and try to make it my best day ever."

He loves serving others, helping those in need and making people better in same way.  

Dr. Thomas Bigham, Medical Director
Terral Bassett, Director of Nursing

Terrel graduated from Dixie College in Nursing in 2000 and is an RN and WCC, Would Care Certified. She was chosen as the top registered nurse in St. George, Utah by the International Nursing Association. She also owned and manages Ridgeview CNA School in St. George.

She loves camping, fishing, 4-wheeling, and teaching. She has been married for 20 years, has 4 children and 2 grandsons, who are the love of her life. 

"I love being a nurse and caregiver. I love to help others. I love being the Director of Nursing and working at Seasons. This is an amazing place. I feel blessed daily, working side-by-side with all of my co-workers. My favorite word  is empathy. I love teaching it to others. I love being a mentor, teacher, advocate, boss, learner, healer, friend, listener and empathetic."

David Gray, Occupational Therapist

David is a graduate of Utah State University with a Bachelor of Science and the University of Utah with a Master's Degree in Occupational Therapy.

He has been married for 14 years and has 2 children. 6 and 12. He loves football, baseball, golf, pickleball, hiking, and camping with family and friends. His new favorite hobby is ice fishing.

"I love to help people feel better and live a more full, healthy and safe life."

Occupational Therapists help people develop skills for daily living such as:
  • Bathing, dressing, grooming, toiletings, eating, etc
  • They also help with instrumental activities such as managing shipping, cooking, medications, housework, laundry, driving, using public transportation, using the phone, and finances so their clients can participate more fully in the life they choose or prevent a disruption in their day to day lives.

Matthew Hess, Speech Pathologist

Matt graduated with honors from the University of Utah with a Bachelor degree in Speech and Hearing Science and from Idaho State University with a Master's degree in Speech Language Pathology. He has been in this field for 18 years. 

Matt loves chocolate, exercising, hiking and playing with his dog. He loves seeing his patient's personalities and seeing them improve.

He helps patient's with:
  • Swallowing problems
  • Communications problems 
  • Cognitive problems
Ashley Davis, Health Unit Coordinator

Carl Green , MDS Coordinator
Picture
Carl is the Nurse Assessment Coordinator. He is responsible for submitting resident health information to the government. 

"I enjoy being able to bring support and happiness to others. I enjoy hearing about the lives others have led."

Carl as a BS in Nursing, RN from Grand Canyon University in Arizona. He has been married to Diana for 17 years, has 7 children and 4 grandchildren. 

He served a mission for the Church of Jesus Christ of Latter-day Saints in St. Louis, MO. 

Through birth and marriage, he has 20 siblings.

he enjoys videos, reading, raising chickens, baking camping and cleaning. his dream job is to become an author of fantasy and sci-fi books. 
Yanely Santos, Dietary Coordinator

Yanely has worked in the dietary department for 21 years and at Seasons for 14 years. She is currently attending school to become a Certified Dietary Manager. Yanely loves to camp, hike, and dance. She enjoys seeing the residents happy with their meals.
  • She assesses residents so that her staff can help with dietory needs
  • She teaches different menus to her staff
  • Looks for ways to prepare better quality meals
Jessica Mock, CNA Supervisor

Jessica received her CNA license in 2007 and has worked at Seasons for 13 years. She has been CNA Coordinator for over 7 years. She enjoys camping, fishing, spending time with family and friends. She has a 12 year-old, charming daughter.

"There is nothing more fulfilling than helping others, and knowing at the end of the day you made a difference for someone. Our residents are all a part of my family and I love them all dearly."
Tawndi Barrett, Billiing Manager

Tawndi is a CNA and began her career at the old Porter's (now Seasons) in 2002. She became transport manager for 6 years until 2019 when she became the Billing manager. 

Her Role at Seasons has many layers;
  • Computing balances
  • Recording billing payments
  • Compiling account statements
  • Billing Medicare and Medicaid
  • Check Insurances for admissions and therapies
  • Collecting card payments, maintaining up-to-date records of patient's demographics
  • Process bills for third party insurances
Tawndi has been married for 13 years. Together they have 6 children, one of whom is a CNA at Seasons. She was born and raised in Oregon and lived 2.5 years in the Cayman Islands where her husband was raised. She loves to sew, craft, go on road trips, camp, but her favorite is fishing. 
Sara Kelley, Resident Advocate

Sara has 4 children, one of which is a CNA at Seasons. She collects funny hats that keep the residents entertained. She likes photography, scenic drives, her ferret Jerry, and 2 dogs Marley and Roxie. 

"I love knowing that I'm helping in our facility and in the community."

Her many responsibilities include:
  • Addressing all admission questions
  • Working with Social Workers, doctors and hospitals
  • She specializes in helping with insurance questions and needs including Medicare and Medicaid
  • She aligns discharges and makes sure that Home Healthcare is received if a resident returns to their home
  • She works within the facility to assist in solving any problems and concerns for residences and their families, whether that is resolving financial issues, grievances or other issues. 
Russell Tolbert, Physical Therapist

 Russell graduated from the University of Central Arkansas with a degree of Physical Therapy. Over the 28 years at a Physical Therapist he has worked in a variety of settings including impatient acute hospital care, acute rehab, transitional care unit, home health and skilled nursing rehab. The last 14 years had been spent working in skilled nursing facilities which is the setting he has found to be the most rewarding. 

Physical Therapy focuses on the treatment of disease, injury or the prevention of loss of mobility through wellness programs using interventions to improve range of motion, strength, posture, endurance, balance, coordination, joint mobility and alleviating pain to help an individual regain their independence with functional mobility skills. 

Physical Therapists are trained to assist in the following areas: ​
  • Gait and stair assessment and training
  • Balance assessment and training with fall risk management
  • Therapeutic exercise programs to improve range of motion, strength and endurance
  • Pain management with the use of modalities including massage, heat/cold therapy, ultrasound, electrical stimulations and manual techniques
  • Home assessments to ensure a safe return home, alone or with family, along with recommendations for home modifications or equipment needs
  • Patient and family education to ensure safety with post treatment programs when discharged home

Marty Maggard, Maintenance Coordinator
Picture
Marty has worked in Mechanical Maintenance all of his adult life. He has worked at Seasons for 9 1/2 years. He keeps our building in excellent working condition to meet the Federal codes and guidelines. His main focus at our facility is life safety. He fixes everything and anything. We couldn't do without him. 

Marty has been married to Debbie, (who works in Recreational Therapy) for 38 years, has  children and 6 grand-children.

His hobbies include fishing, hunting and metal detecting. 
Lisa Paniet, Transportation Manager

Lisa is a CNA and wears many hats at Seasons, assisting with anything that needs to be done. Lisa Loves to smile and laugh. Her phone ringer is a mooing cow. Coffee makes her so happy. She loves camping, kitting, sewing, coloring and watching TV. She was born and raised in Illinois. She has been married for 26 years. She says that "I love my family with all my heart~ Smiling and hugging people every day fills my heart and soul."

The Transportation Department is responsible for the following:
  • Scheduling doctor and dentist appointments as well as any other health care appointments
  • Transport residents to and from appointments and remain with them
  • Pick up new residents
Mary Wilde, Restorative

Mary is an amazing CNA at Seasons. She loves to hike, run and read.

"I love working in a field where kindness and compassion are highly valued. I truly admire and respect all of the staff at Seasons for the love and care they show for our residents." 

Under the Restorative Nurse, Carl Green, mary and Corbin, Restorative CNAs, work with residents through the Restorative program to increase and maintain their independance, promote safety, preserve function, maintain mobility, increase self-esteem, and minimize deterioration. The individual programs are desigd to improve or maintain the functional ability of residents in order for them to acheive their highest level of wellbeing possible.

We offer a range of restorative programs including:
  • Passive Range of Motion (PROM)
  • Active Range of Motion (AROM)
  • Splint or Brace Assistance
  • Transfers
  • Ambulation
  • Dressing and Grooming
  • Eating and Swallowing
Jane Lasko, Office Manager

Jane has worked as the Office Manager at Seasons for 6 1/2 years. She keeps our facility running smoothly and is the best of the best at her job. 

"I love working at Seasons. I consider is a gift from Heaven to be able to associate with the Seasons Family."

She's been married for 56 years and has 5 children and 16 grand-children. ("Some of my kids are slackers in that department.")

Her hobbies are counted cross sticking, gum paste flower making, and quilting. She made the beautiful picture above our mantel in the entryway.

Michelle Anderson, Recreational Therapy Director
Michelle has been a licensed Theropeutic Recreational Technician (TRT) for 23 years, 9 of which have been at Seasons Health and Rehab. She loves spending time with family, hikinig, playiing games, making up stories, listening to up-lifting music, reading the scriptures, being with her dogs, gardening, antique shopping, thrifting and re-selling, and creating. 

"My job is my life calling. I have found joy from being able to serve those around me. I have been humbled over the years as I have served so many residents. They are my family and I love each and everyone of them and am so blessed to have them in my life."
